Health & Wellness Case Manager – FT

We are seeking a full-time Health & Wellness Case Manager to monitor client care and provide health and wellness case management as part of a holistic, personalized treatment plan for women in recovery.

Due to licensing regulations and supervision requirements, this position is open to FEMALE candidates only.

 

The successful House of Hope applicant will:
  • Coordinate care and resources with the HOH staff and community health facjlities
  • Maintain accurate and organized health records for each client
  • Audit processes, records, and all documents related to client healthcare
  • Facilitate prescriptions/medications
  • Practice current wellness and infection control policies and procedures
  • Schedule medical appointments and coordinate transportation with our client team
  • Complete health and wellness assessments for each client
  • Track high-risk pregnancies and coordinate care with client’s OBGYN
  • Teach individuals/groups positive health strategies, especially as it relates to women in recovery
  • Manage referrals to multiple agencies to maintain relationships with partners, vendors, and agencies
  • Identify clients at risk for adverse outcomes; manage crisis incidents as needed
  • Coordinate discharge plans for health and wellness services
  • Be on call, as needed, for healthcare support
  • Be creative in promoting new ways to improve health and wellness for women, children, and the staff of HOH
  • Manage Covid-19 policies and procedures for all HOH clients
  • Other duties and assignments, as needed

  • Due to licensing regulations and supervision requirements, this position is open to FEMALE candidates only
  • Healthcare experience or bachelor’s degree in healthcare-related field
  • Two (2) years of experience in a healthcare or clinically related area
  • Team leadership, relationship building, critical analysis, and written and verbal communication skills
  • Computer skills, data collection, and data documentation
  • Basic knowledge of healthy lifestyles in recovery
  • Provide care to populations served in a professional and strengths-based manner
  • Independent judgment to assess and meet client needs
  • Have meaningful, outcome-oriented dialogue with clients, families, ancillary departments, healthcare and community agencies, and third-party payors
  • Provide transportation for site visits
  • Build relationships with women in crisis and in recovery
  • Communicate effectively and in an organized manner with the many parties involved regarding each family’s healthcare plans and needs

 

Education Required:
  • Bachelor’s degree in healthcare-related field OR experience in healthcare-related industry

 

Hours/Schedule:
  • Fulltime, 40 hours/week
  • Mon-Fri, 8:30am – 5:00pm
  • May require occasional evening or weekend for on-call needs

 

Wages/Benefits:
  • $18/hour
  • Full benefits package, including:

Agency Requirements:
  • This is a non-smoking work site
  • Must be +21 years old
  • Must be able to pass background check and drug test
  • If in recovery, must have at least two years of sobriety
  • Must be vaccinated against COVID-19 and be willing to be seasonally vaccinated against the flu
  • Must have personal reliable transportation and UT driver’s license
