How to Prepare for Admissions at House of Hope
|Please call (801) 487-3276 x1206 for a SUD assessment with House of Hope or you can call Assessment and Referral Services (ARS). You don’t need insurance for an assessment.
|If you received an assessment outside of House of Hope, call our Admissions Specialist at (801) 487-3276 x1233, between Monday and Friday from 8:30 am to 5 pm, and let them know that you’ve completed an assessment.
|We’ll call you to schedule an intake appointment once we receive your assessment. You are welcome to call us any time with questions you may have. Please note that House of Hope is currently unable to take walk-ins.
|Gather needed documents. When you come in for your intake appointment, you’ll need your Utah ID or driver’s license, birth certificate, and social security card, if you have them. Need help getting these items? Click below!
